ABSTRACT

Os debates sobre o acolhimento das diversidades e a inclusão de grupos populacionais em maior situação de vulnerabilidade na área da saúde têm sido crescentes em âmbito nacional e internacional. No Brasil, a trajetória da Nutrição Inclusiva foi impulsionada no início na década de 2000, e atualmente é definida como a forma de exercer a profissão na qual o nutricionista utiliza todos os recursos disponíveis para promover a inclusão, a saúde e a segurança alimentar e nutricional de pessoas em situação de vulnerabilidade, por meio de ações oportunas e ajustadas às singularidades desses sujeitos, tendo como base o acolhimento, o respeito à diversidade, a acessibilidade e o estímulo à autonomia. Atravessa todas as áreas de atuação e cenários de prática do nutricionista, com foco na produção de novos modos de cuidar e novas formas de organizar o trabalho que visem à segurança alimentar e nutricional e à justiça social. Assim, os saberes que vêm sendo construídos nessa direção devem ser incorporados pelo profissional desde a sua formação até a sua prática cotidiana, visando beneficiar direta e/ou indiretamente a saúde e a nutrição dos grupos populacionais em maior situação de vulnerabilidade.


Debates on embracing diversity and the inclusion of more vulnerable populations in the health area have been growing at the national and international levels. In Brazil, the trajectory of Inclusive Nutrition was pushed forward at the beginning of the 2000s and is currently defined as a practice in which the nutritionist uses all available resources to promote inclusion, health, food safety, and nutrition of vulnerable populations through timely actions adjusted to their singularities, based on acceptance, respect for diversity, accessibility, and encouragement of autonomy. It extends over all the areas of action and practice of the nutritionist, focusing on the production of new ways of caring and of organizing work aimed at food and nutritional security and social justice. Thus, the knowledge that has been built in this direction must be incorporated by professionals, from their training to their daily practice, aiming to directly and/or indirectly benefit the health and nutrition of vulnerable populations.

ABSTRACT

La telemedicina, a través del uso de plataformas de videoconferencias por Internet, se encuentra cada vez más extendida en Chile, debido a la pandemia de COVID-19 y las restricciones para controlar el virus. El nutricionista no es ajeno a esta situación y ha tenido que modificar las consultas para dar respuesta a esta nueva realidad. Sin embargo, no existe suficiente información respecto al nivel de satisfacción de los usuarios de la telemedicina, que emplea este profesional. El objetivo de la investigación fue describir la percepción de la satisfacción usuaria relacionada con las consultas nutricionales mediante la telemedicina, realizadas durante la pandemia. Se realizó un estudio observacional, descriptivo y transversal. Se reclutaron 62 pacientes de Santiago de Chile, quienes fueron atendidos en una consulta nutricional por videoconferencia. Se diseñó un cuestionario online constituido por seis ítems para determinar la aceptabilidad de la consulta nutricional por telemedicina. El rol del nutricionista en la consulta nutricional por videoconferencia tuvo un grado de satisfacción superior al 70 por cientoEl 62,9 por ciento de los encuestados prefiere que la consulta nutricional sea combinada. Solo el 50 por ciento señala que la plataforma de videoconferencia zoom favorece la relación nutricionista-paciente. La consulta nutricional por telemedicina permite controlar el estado nutricional del paciente sin necesidad de asistir de manera presencial, lo que mejora el acceso a la atención. Existe un nivel elevado de aceptación por parte de los pacientes que han recibido atención nutricional por telemedicina(AU)


Telemedicine, through the use of Internet videoconferencing platforms, is increasingly widespread in Chile, as a consequence of the COVID-19 pandemic and the restrictions to control the virus. The nutritionist is no stranger to this situation and has had to modify consultations to respond to this new reality. However, there is insufficient information regarding the level of satisfaction of telemedicine users employed by this professional. The objective of the research was to describe the perception of user satisfaction related to nutritional consultations through telemedicine, carried out during the pandemic. An observational, descriptive and cross-sectional study was developed. Sixty-two patients were recruited from Santiago, Chile, who were attended in a nutritional consultation by videoconference. An online questionnaire consisting of six items was designed to determine the acceptability of the nutritional consultation by telemedicine. The role of the nutritionist in the nutritional consultation by videoconference had a degree of satisfaction higher than 70percen. 62.9percentof the respondents prefer the nutritional consultation to be combined. Only 50percent indicated that the zoom videoconferencing platform favors the nutritionist-patient relationship. The telemedicine nutritional consultation makes it possible to monitor the patient's nutritional status without the need to attend in person, which improves access to care. There is a high level of acceptance by patients who have received nutritional care via telemedicine(AU)

ABSTRACT

RESUMEN La educación alimentaria nutricional es la combinación de estrategias educativas, acompañadas de apoyos ambientales, diseñadas para facilitar la adopción voluntaria de elecciones alimentarias que conducen a un estado óptimo de salud y bienestar. La educación alimentaria nutricional tiene tres componentes: 1) motivación, 2) acción y 3) ambiente. Para elegir la técnica de educación adecuada para cada persona, se deben considerar las características psicoemocionales individuales, especialmente las motivaciones para el cambio y el tipo de personalidad, ya que ambos han mostrado ser buenos predictores de la conducta alimentaria. Dentro de los recursos educativos posibles de utilizar para realizar la educación en alimentación nutricional se encuentran indicaciones verbales, material escrito, uso de internet, dispositivos portátiles, aplicaciones de teléfonos inteligentes, e incluso la clase de cocina. La evidencia demuestra que la educación alimentaria nutricional impacta favorablemente la adherencia a las intervenciones nutricionales, lo cual a la vez se ve reflejado en efectos positivos en la salud.


ABSTRACT Food and nutrition education is the combination of educational strategies, accompanied by environmental supports designed to facilitate voluntary adoption of food choices conducive of health and well-being. It has a motivational phase, an action phase, and an environmental component. To choose the right educational technique for each patient, their individual psychoemotional characteristics must be considered; especially what their motivations for change are, as well as their personality type, because both are good predictors of food behavior. Among the resources that can be used to perform nutrition education are verbal indications, written material, the internet, mobile devices, smartphone applications, and even the kitchen as a place for education. Evidence shows that nutrition education has a favorable impact on diet adherence in different conditions that have an important nutritional component, such as celiac disease, inflammatory bowel disease, diabetes and obesity, which at the same time produces positive health outcomes.

RESUMEN Las dietas vegetarianas pueden proveer beneficios para la salud, prevención y tratamiento de enfermedades, siendo adecuadas en todas las etapas del ciclo vital cuando es correctamente asesorada por un profesional especialista en nutrición, entre los que se encuentran las nutricionistas, a fin de prevenir déficits en nutrientes críticos. Debido a esto, resulta relevante identificar la percepción que elaboran sobre el rol del nutricionista las personas vegetarianas que no han contado con asesoría y planificación nutricional profesional. Se realizó un estudio cualitativo centrado en los relatos de vida, donde participaron 10 personas vegetarianas residentes de Coquimbo y La Serena durante el año 2019, seleccionados mediante un muestreo bola de nieve, lo que implicó producir información cualitativa a través de una entrevista semi estructurada. Posteriormente se utilizó análisis cualitativo de contenido, producto de este análisis surgen dos grandes categorías. Como primera categoría el rol biopsicosocial del profesional nutricionista y segunda categoría sugerencias desde los vegetarianos para la atención. Del análisis se destaca la importancia del profesional nutricionista en el abordaje de personas vegetarianas o veganas, quienes esperan de los profesionales un vínculo efectivo con su comunidad mediante instancias de educación alimentaria, asesorías a bajo costo, el desarrollo de competencias técnicas y personales tales como, la actualización continua y la atención empática. Los hallazgos de este estudio pueden ser tomados como guía para la formación y actualización de profesionales, además de proponer medidas nutricionales en el campo de la salud pública y atención contextualizada a personas vegetarianas.


Abstract Vegetarian diets can provide benefits for health, prevention, and treatment of diseases and are appropriate at all stages of the life, when properly advised by specialists in nutrition, to prevent deficits in critical nutrients. Because of this, it is relevant to identify the perception of the role of the nutritionist among vegetarians who have not received professional nutritional advice and planning. A qualitative study focused on life stories was conducted, where 10 vegetarians living in Coquimbo and La Serena participated during 2019, selected through snowball sampling, which involved producing qualitative information through a semi-structured interview. Subsequently, qualitative content analysis was used, and two main categories emerged from this analysis. The first category was the biopsychosocial role of the professional nutritionist, and the second category was suggestions from vegetarians for care. The analysis highlights the importance of the professional nutritionist in the approach to vegetarians or vegans, who expect from professionals an effective link with their community through instances of food education, low-cost advice, the development of technical and personal skills such as continuous updating and empathetic care. The findings of this study can be taken as a guide for the training and updating of professionals, in addition to proposing nutritional measures in the field of public health and contextualized care for vegetarians.

RESUMEN El propósito del presente ensayo es definir estándares de práctica para el nutricionista clínico con el objetivo de determinar un Gold Standard que permita un abordaje sistematizado de pacientes en establecimientos de salud. Se analizó la evidencia nacional e internacional, desde el año 2005 hasta el 2019, en cuanto a estándares de práctica o funciones del nutricionista clínico, considerando población objetivo y rendimiento, es decir, la cantidad de pacientes por Nutricionista o el tiempo asignado por atención. De los resultados obtenidos, destacaron un total de 11 instituciones, organizaciones o programas de salud que describieron estándares de práctica o funciones del nutricionista, donde todos indicaron población objetivo y sólo 9 rendimiento. En la actualidad, la ASPEN-ADA, ha sido las única entidad que ha publicado estándares de práctica para nutricionistas clínicos, clasificados además en niveles de competencia técnica, aunque sin indicar rendimiento. En síntesis, el presente trabajo determina los estándares de práctica publicados por la ASPEN-ADA como el Gold Standard de práctica en nutrición clínica, sirviendo como herramienta diagnóstica para determinar un índice de nutricionistas/paciente pertinente.


ABSTRACT The purpose of this review is to define standards of practice for the clinical nutritionist, with the objective to determine a gold standard of a patient care in order to systematize approaches in health establishments. This included the analysis of national and international evidence, from 2005 to 2019 in terms of standards of practice or functions of the clinical nutritionist, considering the target population and performance, the number of patients per nutritionist or the time allocated for the activity. The results obtained were from a total of 11 institutions, organizations or health programs that described practice standards or functions of the clinical nutritionist, in which all of them indicated the target population and 9 mentioned performance. The ASPEN and ADA, have been the only entities that have published practice standards for clinical nutritionists have also classified levels of technical competence, without indicating performance. In summary, the present work determines the standards of practice published by the ASPEN-ADA as the gold standard of practice for clinical nutrition, serving as a diagnostic tool to determine a pertinent index of nutritionists/patient.

ABSTRACT Background: Body composition assessment in breast cancer survivors (BCSs) is essential to plan feasible dietary strategies for sarcopenic obesity prevention. Objective: Studying the effect of an individualized nutrition intervention according to socioeconomic status and grocery shopping behavior on BCSs relative fat mass (RFM). Methods: BCSs attending an academic medical center were studied; participants saved all 1-week supermarket tickets and answered a grocery shopping consumer preference survey. RFM was assessed at baseline and after the 3-month nutrition intervention. Nutrition plans were based on the dynamic macronutrient meal-equivalent menu method (MEM) and dietary guidelines for BCSs. Results: Thirty-three BCSs completed the study and 91% of them presented obesity or overweight at baseline. After the intervention, BCSs lost 1.6 kg (p < 0.01) of body weight, 1.8 kg (p < 0.01) of RFM, 3 cm (p < 0.01) of waist circumference, and 2.4 cm (p < 0.01) of hip circumference, while no changes were observed in fat-free mass (p = 0.6) and arm bone-free muscle area (p = 0.7). Conclusions: RFM and body weight in breast cancer survivors decreased after an individualized nutrition intervention according to socioeconomic status and grocery shopping consumer behavior. Based on the participants’ food preferences and consumer behavior, plant-based protein diet plans cost less than the animal-based protein diet plans.

This study aims to analyze the scientific production of Nutrition courses on knowledge core Food and Nutrition in Collective Health (FNCH) and its associations with institutional characteristics and advisor's academic degree. A bibliometric study was performed on 195 final projects of five nutrition courses in Rio Grande do Nortestate (Brazil) from the year 2013. Information related to higher education institutions and academic degree of advisor were collected. From the reading of sections of final projects, screening was performed on final project involvement with knowledge core FNCH, being collected general methodological characteristics, classification on sub cores of FNCH, and theme approached. Pearson's chi-square test was used with a significance level at p-value ≤ 0.05 and 95% confidence interval in a univariate and bivariate way.From the total of analyzed final projects, 54 (27.7%) were related to knowledge core FNCH, prevailing final projects with a quantitative approach (61%), presented as scientific articles (57%), and performed in public services (45%). There was an emphasis on the sub-core of Nutritional Epidemiology (63%) [p < 0.001], with the theme Nutritional Assessment (57%) [p < 0.001]. There was no significant association between FNCH final projectsproportion and institutional characteristics or advisor's academic degree. Thinking about nutritionists' practice, limitations were identified on FNCH scientific production, with an emphasis on nutritional diagnosis researches, with less involvement in public health policies and human and social sciences.

Introdução: Questões sobre sustentabilidade no Programa Nacional de Alimentação Escolar estão cada vez mais prementes e o profissional nutricionista é chave nesse processo. Objetivos: Verificar o conhecimento, as ações e a importância dada às dimensões das dietas sustentáveis de nutricionistas que atuam na alimentação escolar no sudoeste e oeste do estado do Paraná. Métodos: Trata-se de uma pesquisa transversal, feita a partir de questionário on-line sobre conhecimentos, importância e ações de sustentabilidade, respondido por 32 nutricionistas. Foi utilizado software PSPP para análises descritivas e teste de Mann Whitney. Resultados: Verificou-se que a maioria (62,5%) não teve formação acadêmica sobre o assunto e pondera não ter conhecimentos suficientes sobre dietas sustentáveis. Embora todos considerem de muita ou média importância os aspectos de sustentabilidade, a dimensão ambiental foi a que obteve menor percentual de "muita importância" (75%). Em relação às ações, observou-se que as que obtiveram maior aderência dos nutricionistas eram as relativas ao cumprimento de legislações e à economicidade. Encontrou-se associação significativa (p<0,05) entre a importância dada à dimensão econômica e a quantidade de ações propositivas de sustentabilidade realizadas pelos nutricionistas. Os dados demonstram que os profissionais que realizam mais ações sustentáveis tendem a ser aqueles mais preocupados com os aspectos econômicos do que propriamente com aspectos ambientais. Conclusão: Há necessidade de formação e sensibilização destes profissionais sobre dietas sustentáveis para que possam compreender melhor o que isso abrange e tenham mais clareza sobre as ações que devem realizar para alcançar a sustentabilidade, além de mais recursos financeiros para tal.


Introduction: Sustainability issues in the National School Feeding Program (PNAE, in its acronym in Portuguese) have been increasingly pressing, and nutrition professionals are key in this process. Objectives: To verify knowledge, actions and importance given to the sustainable diets dimensions by nutritionists working in school feeding programs in the southwestern and western State of Paraná, Brazil. Methods: This is a cross-sectional study based on an online questionnaire on knowledge, importance given to, and actions regarding sustainability, which was answered by 32 nutritionists. The PSPP software, for descriptive analysis, and the Mann Whitney test were used. Results: It was found that most respondents (62.5%) did not have academic training on the subject and considered they did not have sufficient knowledge about sustainable diets. Although all of them consider sustainability aspects to be very or moderately important, the environmental dimension had the lowest percentage as "very important" (75%). Regarding actions, it was observed that those related to compliance with legislation and economy obtained greater adherence from nutritionists. A significant association (p<0.05) was found between the importance given to the economic dimension and the number of sustainability actions carried out by nutritionists. The data show that professionals who carry out more sustainable actions tend to be those more concerned with economic aspects than with environmental aspects. Conclusion: There is a need for training and awareness about sustainable diets for these professionals so that they can understand more deeply what the topic encompasses and have more clarity about the actions they must take to achieve sustainability, as well as more financial resources for it.

ABSTRACT Objective To make a critical and comparative analysis of curricula of Brazilian and Portuguese higher education institutions in terms of clock hours and semester distribution of food and nutrition education in undergraduate nutrition programs, also assessing the main differences among courses classified into thematic axes and professional practice areas. Methods The curricula of fifteen Brazilian and eleven Portuguese nutrition programs were collected and classified into thematic axes and professional practice areas with the method of Document Analysis. Next, we performed statistical analysis regarding the total and proportional clock hours of instruction and their semester distribution to assess the differences between the two countries. The variables of interest were the hours of Food and Nutrition Education and their semester distribution. Results The Food and Nutrition Education axis was the second smallest one, with statistically significant differences among the axes (2.2% of curricula; p<0.001). Brazilian higher education institutions showed greater total clock hours of Food and Nutrition Education (p=0.018), Human and Social Sciences (p=0.003), Public Health (p<0.001), as well as a wider dispersion and lower relative weighted mean for the semester offering of courses (p=0.004) of Food and Nutrition Education courses. Portuguese higher education institutions showed greater total and proportional clock hours of instruction for Exact Sciences (p<0.005; p=0.001, respectively) and more proportional hours of Biologic and Health Sciences (p<0.001). Conclusion Our study found a reduced presence of the area of Food and Nutrition Education in the undergraduate training of nutritionists in both countries.


RESUMO Objetivo Realizar uma análise crítica e comparativa das horas de instrução e distribuição semestral da Educação Alimentar e Nutricional nos cursos de graduação em nutrição. Além disso, analisar as principais diferenças entre disciplinas classificadas em Eixos Temáticos e em Áreas De Atuação Profissional de currículos de cursos de nutrição das Instituições de Ensino Superior brasileiras e portuguesas. Métodos Os currículos de quinze instituições brasileiras e onze portuguesas foram coletados, e o método Análise Documental foi usado para classificar as disciplinas de cada curso em Eixos Temáticos e em Áreas de Prática Profissional. Em seguida, foi feita uma análise estatística das horas de aula totais e proporcionais e do semestre de oferta, para verificar a diferença entre os países. As horas de instrução de Educação Alimentar e Nutricional e a distribuição semestral foram as variáveis de interesse. Resultados O eixo Educação Alimentar e Nutricional foi o segundo menor, com diferença estatisticamente significativa entre eixos (2,2% dos currículos; p<0,001). As Instituições de Ensino Superior brasileiras apresentaram maior carga horária absoluta para Educação Alimentar e Nutricional (p=0,018), Ciências Humanas e Sociais (p=0,003) e Saúde Pública (p<0,001), e maior dispersão e menor média ponderada relativa de localização do curso (p=0,004) para as disciplinas de Educação Alimentar e Nutricional. As Instituições de Ensino Superior portuguesas apresentaram maior carga horária absoluta e proporcional em Ciências Exatas (p<0.005; p=0,001 respectivamente) e maior proporcional em Ciências Biológicas e da Saúde (p<0.005). Conclusão Este estudo encontrou uma presença reduzida da área de Educação Alimentar e Nutricional na formação de nutricionistas em ambos os países.

RESUMEN El trabajo del nutricionista clínico implica realizar una evaluación y diagnóstico nutricional basados en el pensamiento crítico y razonamiento clínico, para una atención nutricional adecuada, y tomar una decisión de intervención basada en la evidencia científica disponible. Luego, se debe monitorizar los resultados de la intervención, para generar un impacto a corto y largo plazo en las patologías que requieren tratamiento nutricional. Para lograr esto, es necesario implementar en los programas curriculares del pregrado un proceso estructurado de enseñanza que siga una secuencia lógica dependiente del nivel de formación, para adquirir habilidades del trabajo clínico en nutrición. El siguiente ensayo tiene por objetivo realizar un análisis de la literatura con respecto a la adquisición de habilidades del pensamiento crítico y razonamiento clínico, junto con conocer los modelos de atención nutricional para generar una propuesta de atención nutricional para aplicar en la práctica clínica.


ABSTRACT The activities of a clinical dietitian involve nutritional assessment and diagnosis based on critical thinking and clinical reasoning. The objective is to offer proper nutritional care and deciding on interventions based on available scientific evidence. The results of the intervention should be monitored to generate short- and long-term impact on the pathologies that require nutritional treatment. To achieve this goal, it is necessary to implement a structured teaching process in the undergraduate program that follows a logical sequence depending on the level of training. The following essay aims to conduct an analysis of the literature regarding the acquisition of critical thinking skills and clinical reasoning, together with knowing the models of nutritional care to generate a nutritional care proposal for application in clinical practice.

ABSTRACT

Resumo O nutricionista é um profissional importante para a promoção da saúde individual e coletiva, tendo em vista a realização da segurança alimentar e nutricional (SAN), que focaliza o direito humano à alimentação adequada (DHAA) e se constitui estratégia de ações e políticas que pautam o próprio DHAA, a soberania alimentar e o enfoque sistêmico. Nesse sentido, a formação do nutricionista deve incluir conteúdos que subsidiem a sua tarefa de promoção da alimentação adequada e saudável, comprometida com os princípios do Sistema Único de Saúde, na perspectiva da SAN. Este estudo analisou 27 projetos pedagógicos do curso de nutrição de universidades federais das cinco regiões do país, visando identificar como a SAN aparece nos currículos e se constitui elemento para a construção do perfil profissional. A análise evidenciou que, apesar de a maioria estar em consonância com as orientações das Diretrizes Curriculares Nacionais em relação ao perfil esperado para o egresso, o tema é abordado por um número restrito de disciplinas, apontando baixa representatividade dos conteúdos de SAN em relação a outros conteúdos disciplinares. Conclui-se que o fortalecimento do tema na formação refletirá na atuação do profissional engajado e comprometido com a sua realização no cotidiano.


Abstract Nutritionists are key for individual and collective health promotion focused on achieving Food and Nutrition Security (FNS) policies, which in Brazil focuses on the human right to adequate food (HRAF) and includes guidelines for itself, for food sovereignty, and its systemic approach. The nutritionist's training must include content that promotes adequate and healthy food, following SUS principles from the perspective of FNS. This study analyzes 27 pedagogical projects of the Nutrition course from federal universities in the five regions of Brazil to identify how the FNS appears in Nutrition curricula and functions as an element for constructing a professional profile. The results showed that, although most curricula comply with the national curriculum guidelines regarding the expected profile for graduates, the subject is addressed by a restricted number of disciplines, indicating a low representativeness of FNS content regarding interdisciplinary content. Strengthening the subject during training thus reflects on these professionals' performance in daily life.

ABSTRACT

La pandemia producida por la COVID-19 a consecuencia del virus denominado SARS-CoV-2 o síndrome respiratorio agudo grave, que en diciembre de 2019 se originó en China, ocasionó la suspensión momentánea del planeta y con ella la consecuente afectación de los procesos de producción, distribución, comercialización y consumo de alimentos, que a su vez devino en una problemática sanitaria nutricional con alteración de los patrones de vida cotidianos. Motivado por la situación anterior, se planteó estudiar el cómo la COVID-19, podría afectar la seguridad alimentaria y a los comedores escolares desde la perspectiva del nutricionista. Para ello se revisaron documentos de acceso público de organismos autorizados disponibles, entre marzo y mayo del 2020. La información se organizó, para abordar cuatro grandes áreas, relacionadas con las dimensiones de la seguridad alimentaria: la producción, la distribución, los ajustes en el consumo de alimentos y los riesgos relacionados con la alimentación y nutrición. Esta revisión finaliza, con el aporte de la ventana de oportunidades que emerge para el nutricionista en los servicios de alimentación y especialmente en los comedores escolares, que abarcan la importante población infantil ante los cambios que serán necesarios implementar para los ajustes que la etapa post pandemia seguramente demandará. Una buena alimentación, representa no solo un cambio del presente sino la herencia para nuestros hijos y las futuras generaciones. Invertir en los niños, significa invertir en la salud futura de las naciones, siendo esta una intervención segura, saludable, divertida y deliciosa(AU)


The COVID-19 Pandemic virus or SARS-COV-2, is a severe acute respiratory syndrome which originated in China on December 2019. In consequence the life style patterns of consumption, production and distribution of foods have been affected worldwide. This event, created the need to study the Food Security effect in the school restaurants from the perspective of a nutritionist. To do so, I reviewed public documents, of March to May 2020, from authorized organizations. The information was classified in four areas of Food Security: production, distribution, consumption and the risk related to food and nutrition. At the end of the review it was found that there are many opportunities for nutritionist in Food Services, specifically school restaurants. We need to implement changes according to the needs of students after COVID-19 has subsided. A good diet represents not only a change of the present, but of the inheritance for our children, and future generations. Investing in children means investing in the future health of nations as this is a safe, healthy, fun and delicious way to intervene in the health of the children(AU)

Antecedentes: actualmente, Chile tiene un déficit de dietistas clínicos y se desconoce si este déficit está afectando la prescripción de indicaciones dietéticas adecuadas para pacientes hospitalizados. Objetivo: evaluar el nivel de participación del nutricionista clínico en la prescripción dietética en los hospitales públicos y privados del territorio nacional, entre los meses de agosto y octubre del año 2016. Materiales y métodos: estudio exploratorio, descriptivo; se invitaron a participar a 360 nutricionistas, contestaron 110 nutricionistas clínicos. Se aplicó un cuestionario de 21 preguntas, enviado vía correo electrónico. Resultados: las horas diarias dedicadas al servicio clínico presentaron una mediana de 6 horas, el 41,8 % de los encuestados tenía entre 26 y 50 pacientes a su cargo. El 99 % de los nutricionistas puede sugerir cambios en la prescripción dietética; sin embargo, un 45 % de las sugerencias realizadas al médico para modificar regímenes son registradas siempre en la ficha clínica. La participación en la prescripción dietética al inicio del tratamiento corresponde a un 9,1 %, en la evolución es de un 32,7 % y en la prescripción realizada al alta es de un 56,4 %. A más años de experiencia y más horas dedicadas al servicio se presenta un significativo mejor puntaje en la prescripción dietética (p<0,05). Conclusiones: el nivel de participación del nutricionista clínico es medio, los nutricionistas con más años de experiencia y mayor número de horas dedicadas a los pacientes presentan mayor prescripción dietética.


Background: Currently Chile has a deficit of clinical dietitians and it is unknown if this deficit is affecting the prescription of adequate dietary indications to hospital patients. Objective: Evaluate the participation level of clinical dietitians in the adequate prescription of dietary indications in public and private hospitals nationally, between August and November 2016. Materials and Methods: Exploratory descriptive study. 360 dietitians were invited to participate, and 110 accepted. A 21-question self-response questionnaire was sent to participants via email. Results: Daily hours dedicated to clinical services was a median of 6 hours among participants, 41.8% of whom had between 26 and 50 patients in their care. 99% of the dietitians are able to suggest changes to the dietary prescriptions; however only 45% of the modifications that are suggested to the doctors are ever recorded in the clinical records. Participation in dietary prescription at the beginning of patient treatment is 9.1 %, throughout the hospital stay is 32.7 %, and prescriptions made at discharge correspond to 56.4 %. More years of experience and more hours dedicated to clinical service correspond to a significantly better percentage in dietary prescriptions administered (p<0.05). Conclusions: The level of dietitian participation is average; the dietitians with more years of experience and higher number of hours dedicated to patient care present better dietary prescription to hospital patients.

Using internetstructured questionnaire focusing on dietary habits before andafter college education, physical activity and weight changes totest the impact of education on food habits both on the subjectand his/her surrounding environment (family and friends).Results: Increase consumption of fruits, vegetables, darkgreen vegetables, whole grain cereals, dairy products, andwater and increase physical activities, in addition to decreaseconsumption of soda, artificial canned juices, energy drinks,solid fat, added sugar, saturated fat and salt were clear amonggraduates of clinical nutrition departments in comparison toother graduates.Conclusion: Professional nutrition education positively impactfood choices and healthy food habits. This impact is extendedto their families and friends.

Introdução: O estado nutricional e a avaliação do gasto metabólico têm impacto significativo na evolução clínica dos pacientes. Objetivo: Identificar quais métodos de avaliação do estado nutricional e o cálculo da necessidade energética têm sido eleitos na prática clínica de nutricionistas, conhecendo os motivos que influenciaram suas escolhas. Metodologia: Aplicou- -se um questionário semiestruturado, com perguntas quanti e qualitativas, por meio do autopreenchimento. Resultados: A maioria dos entrevistados era do gênero feminino, formados em instituições públicas, com formação complementar. O principal local de atuação foi a internação e o único equipamento comum a todos foi a balança. Os métodos escolhidos por 100% dos profissionais foram o IMC e o método Kcal/Kg de peso. Conclusão: Apesar do conhecimento teórico-prático de diferentes métodos de avaliação nutricional e do cálculo da necessidade energética houve preferência pelos de maior facilidade e praticidade, havendo pouco reconhecimento de suas limitações. Entre os motivos para tais escolhas destacam-se a falta de equipamentos e o número de profissionais por enfermaria. (AU)


Introduction: Nutritional status and the evaluation of metabolic expenditure have a significant impact on the clinical evolution of patients. Objective: To identify which nutritional status and energy need assessment methods have been chosen in the clinical practice of nutritionists, knowing the possible reasons that influenced their choices. Methodology: For the study, a semi-structured questionnaire was developed, with quanti and qualitative questions and the data was collected through self-completion. Results: Most of the interviewees were female, graduated in public institutions and all of them had made complementary studies. The main place of work of the professionals was the hospitalization and the only equipment present common to all nutritionists was the scale. The chosen methods by 100% were the BMI and the Kcal/Kg body weight method. Conclusion: Despite the theoretical and practical knowledge of different methods of nutritional assessment and calculation of the energy requirement, there was a preference for the easiest and most practical methods, with little recognition of their limitations. Other reasons include the lack of equipment and the number of professionals per ward. (AU)

ABSTRACT Objective To carry out a mapping of the historical trajectory of the professional associations of nutritionists (scientific associations, councils and professional labor unions) in Brazil. Methods The study consists of a historical-documentary analysis. To obtain the data, information and documents, the websites of the Federal Council of Nutritionists; Brazilian Association of Food and Nutrition; National Federation of Nutritionists and Nutritionists' Unions were browsed. A systematic bibliographic survey was carried out in the databases of the Scientific Electronic Library Online and National Library of Medicine to retrieve scientific articles about the associations of nutritionists in Brazil over the 80 years of their history, focusing on the last ten years. Results The historical analysis indicates that, over the course of eighty years, there has been strengthening of professional associations of nutritionists and achievements concerning training regulation, professional practice, fields of work and working conditions. The labor reform act of 2017 reduces the rights related to working conditions, creating setbacks in the achievements of nutritionists. Conclusion The professional associations of nutritionists have played an important role in the process of professionalization and legitimacy of training and performance of the profession in the country. There are indications of the unified performance of professional associations, but it is still incipient and scarce. Nowadays, unified, transparent and committed action of these professional associations is imperative for the professionalization and improvements of the working conditions of Brazilian nutritionists.


RESUMO Objetivo Realizar mapeamento da trajetória histórica das entidades de classe do nutricionista (associações científicas, conselhos e sindicatos profissionais) no Brasil. Métodos O estudo consiste em uma análise histórico-documental. Para obtenção dos dados, informações e documentos foram feitas visitas aos sítios eletrônicos do Conselho Federal de Nutricionistas; Associação Brasileira de Alimentação e Nutrição; Federação Nacional de Nutricionistas e Sindicatos de Nutricionistas. Realizou-se levantamento bibliográfico sistematizado nas bases Scientific Electronic Library Online e National Library of Medicine para capturar artigos científicos sobre a temática no Brasil ao longo dos 80 anos de sua história, com ênfase nos últimos dez anos. Resultados A análise histórica da Associação Brasileira de Alimentação e Nutrição, do Sistema Conselho Federal/Conselhos Regionais de Nutricionistas e da Federação Nacional de Nutricionistas/Sindicatos de Nutricionistas indica que, ao longo da trajetória de oitenta anos, houve fortalecimento das entidades de classe, com várias conquistas no arcabouço de regulamentação da formação, exercício profissional, campos de atuação e condições de trabalho. A reforma trabalhista, instituída em 2017, imprime redução de direitos relativos às condições de trabalho, acenando para retrocessos nas conquistas trabalhistas do nutricionista. Conclusão As entidades de classes do nutricionista têm desempenhado relevante papel no processo de valorização e legitimidade da formação e atuação da profissão no país. Há indícios da atuação unificada das entidades de classe, mas ainda incipientes e pontuais. Na atualidade, urge uma atuação unificada, transparente e comprometida destas entidades de classe em busca de valorização da profissão e melhorias das condições de trabalho do nutricionista brasileiro.

ABSTRACT Objective This study sought to highlight the different types of challenges for the integral care of overweight individuals in primary care based on the perception of nutritionists. Methods This is a qualitative study with primary care professionals from the Federal District, Brazil. Semi-structured interviews were conducted to understand the perceptions regarding the organization of integral care. The analysis of the data was based on the theoretical framework Theoretical Domains Framework. Results Challenges related to 10 of the 12 different domains proposed by the theoretical framework were identifi ed. These challenges are related to the capability, opportunity and motivation of workers to act to offer moreeffective care. Conclusion The different nature of the barriers identifi ed were overlapped, which reveal the need for professional qualifi cation along with the consolidation of an appropriate care model for chronic noncommunicable diseases such as obesity.


RESUMO Objetivo Este estudo buscou compreender, a partir da percepção dos nutricionistas, as diferentes naturezas dos desafiospara o cuidado integral dos indivíduos com excesso de peso na atenção básica. Métodos Trata-se de um estudo qualitativo com profissionais da atenção básica do Distrito Federal, Brasil. Foram realizadas entrevistas semiestruturadas, buscando compreender percepções quanto aos desafios para a organização do cuidado integral dos indivíduos com excesso de peso. Para interpretação dos dados, foi utilizada a análise de conteúdo, a partir do quadro teórico denominado Theoretical Domains Framework Resultados Foram identificados desafios relacionados a 10 dos 12 diferentes domínios propostos pelo quadro teórico. Os referidos desafios estão relacionados à capacidade, à oportunidade e à motivação dos trabalhadores em agir, nosentido de ofertar um cuidado mais efetivo. Conclusão As diferentes naturezas das barreiras para a oferta de um cuidado iintegral dos indivíduos com excesso de peso se mostraram bastante imbricadas, apontando para uma necessidade de qualificação profissional de forma conjunta com a estruturação de um modelo de atenção adequado às condições crônicas, como o excesso de peso.

Abstract The objective of this study was to evaluate the food safety performance of food services and its associated factors. The study involved 84 food businesses in the city of Santos as follows: street food kiosks and beach kiosks (street food), commercial restaurants, hospitals and school meal services (institutional food services). A food safety checklist with 81 items was applied. The overall percentage of adequacy was lower in street food kiosk (49.7%) and beach kiosk (62.0%) than in restaurants (69.7%), hospitals (83.9%) and schools (86%). Institutional and commercial restaurants showed higher scores of food safety performance than street food services, mainly regarding risker practices. The variables positively associated with food safety performance in commercial restaurants were: presence of a nutritionist as leader and adequate structure. These results show that street food services need, in a mediate way, the implementation of new strategies and regulations to improve food safety. A nutritionist acting as food safety leader may improve the food safety performance at commercial restaurants.


Resumo O objetivo desse estudo foi avaliar o desempenho dos serviços de alimentação e seus fatores associados. O estudo envolveu 84 serviços de alimentação na cidade de Santos, sendo estes: ambulantes, quiosques de praia (grupo comida de rua), restaurantes (grupo alimentação comercial), hospitais e escolas (grupo alimentação institucional). Uma lista de verificação com 81 itens foi aplicada. Classificações de risco sanitário foram utilizadas para cada item. O percentual geral de adequação foi menor nos ambulantes (49,7%) e quiosques de praia (62,0%) do que em restaurantes (69,7%), hospitais (83,9%) e escolas (86%). Serviços de alimentação institucionais e comerciais apresentaram escores mais elevados no desempenho em segurança dos alimentos do que os pertencentes ao grupo comida de rua nos quesitos de maior risco sanitário. As variáveis positivamente associadas com o desempenho em restaurantes comerciais foi a presença de nutricionista e estrutura adequada. Esses resultados indicam a necessidade de novas estratégias e políticas para os serviços de comida de rua e que o nutricionista atuando como líder de segurança dos alimentos pode melhorar o desempenho de restaurantes comerciais em relação à segurança dos alimentos.

ABSTRACT

Resumo O Programa Nacional de Alimentação Escolar (PNAE) preconiza a promoção de práticas alimentares saudáveis por meio da oferta de refeições juntamente com ações de educação alimentar e nutricional (EAN). O objetivo deste trabalho foi identificar as ações de EAN no PNAE e sua relação com o perfil do nutricionista. Estudo transversal analítico, realizado em municípios de Goiás, Brasil. Dados coletados por telefone, sendo a associação entre a variável desfecho (realização de EAN) e as de exposição (população escolar e atuação do nutricionista) feita por teste qui-quadrado de Pearson, nível de significância de 5,0%. Foram incluídos 214 (87%) municípios, dos quais 91,1% promovem EAN, com frequência predominantemente semestral (25,3% nas escolas e 23,2% nas creches). A presença do nutricionista mostrou-se associada à EAN (p = 0,001). Ações de EAN são executadas na maioria dos municípios, entretanto com baixa frequência. A associação encontrada indica a relevância da presença do nutricionista, em condições que atendam à legislação, possibilitando o cumprimento de suas atribuições, conforme definido pelo Marco Legal do PNAE e do Conselho Federal de Nutricionistas.


Abstract The National School Food Program (NSFP) recommends the promotion of healthy eating habits by offering meals along with food and nutrition education (FNE) actions. The objective of this study was to identify the FNE in the NSFP and its relationship with the nutritionist's profile. It involved an analytical, cross-sectional study conducted in cities in the state of of Goiás, Brazil. Data was collected by telephone, and the association between the outcome variable (achievement of FNE actions) and exposure (school population and nutritionist performance) was done by Pearson's chi-square test of with a 5% significance level. In this study, 214 (87%) municipalities were included, among which, 91.1% promote FNE actions with predominantly biannual frequency (25.3% in schools and 23.2% in day care centers). The nutritionist presence was associated to the FNE (p = 0.001). FNE actions are performed in most municipalities, however with low frequency. The association indicates the relevance of a nutritionist present in conditions that heed the legislation, enabling the performance of their duties, as defined by the Legal Framework of the NSFP and the Federal Council of Nutritionists.

A agricultura familiar rural tem ancorado as famílias produtoras de alimentos por meio do Programa de Aquisição de Alimentos e do Programa Nacional de Alimentação Escolar, com atenção a questão social, além de incentivar e facilitar o escoamento dessa produção. O objetivo desta pesquisa foi verificar o cumprimento da Lei nº 11.947/2009 e regulamentações no Programa Nacional de Alimentação Escolar (PNAE), analisando as características dos documentos publicados entre janeiro e julho de 2013. Foram localizados duzentos e um editais de chamada públicas pesquisados por meio eletrônico, com representação de todas as Regiões do País. Os resultados indicaram características não conformes à legislação vigente para a maioria das Entidades Executoras, com falhas na informação dada ao produtor familiar rural, principalmente quanto ao cronograma de entrega e indicação da prioridade na escolha do projeto de venda. Considerando os resultados, concluiu-se que grande parte dos gestores deixou de colocar em prática a Lei nº 11.947/2009 e regulamentações, com maior proporção de não conformidades nas regiões Centro Oeste, Nordeste e Norte, alertando para a necessidade de fiscalização rigorosa e efetiva às EEx. por parte dos órgãos fiscalizadores para o cumprimento da legislação vigente, assim como para uma melhor orientação aos produtores familiares rurais sobre esse assunto, evitando a continuidade de procedimentos não conformes.(AU)


Rural family agriculture has anchored the food producing families through the Food Acquisition Program and the National School Feeding Program, with attention to the social issue, besides encouraging and facilitating the disposal of this production. The objective of this research was to verify compliance with Law 11.947 / 2009 and regulations in the National School Feeding Program (PNAE), analyzing the characteristics of the documents published between January and July 2013. Two hundred and one public call notices were searched by electronic means, with representation from all Regions of the Country. The results indicated characteristics that do not conform to the legislation in force for most of the Executing Entities, with deficiencies in the information given to the rural family producer, mainly regarding the delivery schedule and indication of priority in the choice of the sales project. Considering the results, the conclusion is that most of the managers failed to put into practice Law 11.947 / 2009 and regulations, with a higher proportion of nonconformities in the Midwest, Northeast and North regions, alerting to the need for rigorous and effective supervision to the EEx. by the enforcement agencies to comply with current legislation, as well as to provide guidance to rural family farmers on the subject, avoiding the continuation of non-compliant procedures.
